Neutrophil to Lymphocyte Ratio (NLR) Calculator

How it works

The formula

NLR = absolute neutrophil count ÷ absolute lymphocyte count, both from the same CBC with differential. Use the absolute counts (K/µL, numerically identical to 10⁹/L) — never the percentages. If your report only shows percentages, multiply each by the total WBC count first.

What the ratio reflects

Neutrophils are the immune system's first responders — they surge with acute inflammation, infection, and physiological stress. Lymphocytes coordinate adaptive immunity and tend to dip under the same conditions. Dividing the two concentrates that seesaw into a single number, which is why NLR is studied as an inexpensive inflammation and immune-balance signal. Evidence level & limitations

The bands are research-cohort conventions — there is no official clinical cut-off for NLR in general health screening, and reference windows vary between studies. A single NLR value is far less informative than a trend, and recent illness, hard exercise, poor sleep, and steroid medications can all shift it within hours to days. How do I calculate my neutrophil to lymphocyte ratio?

Take the absolute neutrophil count and absolute lymphocyte count from the same CBC with differential and divide: neutrophils ÷ lymphocytes. Example: neutrophils 4.0 K/µL with lymphocytes 2.0 K/µL gives an NLR of 2.0. The calculator above does it instantly.

Where do I find these numbers on my CBC report?

Look for the absolute counts — usually labeled NEUT#, ANC (absolute neutrophil count), LYMPH#, or 'abs' — not the percentages. Most reports list both side by side in the differential section. If yours only shows percentages, multiply each percentage by the total WBC count to get the absolute values.

What is a high neutrophil to lymphocyte ratio?

In research cohorts, values above 3 sit above the typical healthy-adult window, and above 5 is a strong acute-phase pattern. There is no official clinical cut-off — recent illness, intense exercise, poor sleep, and steroid medications can all push NLR up temporarily, so context and trends matter more than any single reading.

Can NLR diagnose inflammation or any disease?

No. NLR is a research-grade signal computed from routine blood work — it is nonspecific, meaning many unrelated conditions (and plain physiological stress) move it in the same direction. It is a screening signal that can add context alongside markers like hs-CRP, never a diagnosis. Disease-specific uses of NLR belong with your medical team.

How is NLR different from hs-CRP?
