How it works

The formula

TyG = ln( fasting triglycerides mg/dL × fasting glucose mg/dL ÷ 2 ). The natural log compresses the scale, so small differences at the top end matter more than they look.

Why it works without an insulin assay

Insulin tests are expensive and not standardized across labs. Triglycerides and glucose are on every standard panel. Elevations in both reflect the same underlying energy-handling dysfunction that insulin resistance produces, which is why their combination tracks HOMA-IR surprisingly well.

Limitations

Cut-offs differ across populations and studies — 8.5 is a commonly cited threshold, not a universal one. Recent illness, alcohol intake, and non-fasting samples all distort triglycerides. Use TyG as one signal among several.

Frequently asked questions

What is a normal TyG index?

In research cohorts, values below ~8.0 are typically seen in insulin-sensitive individuals, 8.0–8.5 is intermediate, and above ~8.5 is associated with higher insulin resistance likelihood. Exact cut-offs vary by population and study.

My labs are in mmol/L — how do I convert?

Glucose: multiply mmol/L by 18 to get mg/dL. Triglycerides: multiply mmol/L by 88.57 to get mg/dL. Then use the calculator normally.

How does TyG compare to HOMA-IR?

Both estimate insulin resistance. HOMA-IR needs a fasting insulin assay; TyG uses triglycerides and glucose, which are cheaper and already on most panels. Studies show good correlation between the two, and some find TyG performs as well or better for predicting metabolic syndrome.

Is the TyG index a diagnosis?

No. It is a screening marker derived from routine labs. A high value is a prompt to look deeper — not a diagnosis of any condition. Discuss your full panel with a qualified practitioner.
