How it works
The formula — and the unit trap
This calculator uses the community-standard form: T/E2 = testosterone (ng/dL) ÷ estradiol (pg/mL). Example: T 600 ng/dL, E2 30 pg/mL → 600 ÷ 30 = 20. Because the two inputs use different units, the ratio's absolute number depends on this convention — if you see a T/E2 figure elsewhere, check which units produced it before comparing.
Unit conversions
Testosterone: ng/dL = nmol/L ÷ 0.0347 (e.g. 20.8 nmol/L ≈ 600 ng/dL). Estradiol: pg/mL = pmol/L ÷ 3.671 (e.g. 110 pmol/L ≈ 30 pg/mL). Both values must come from the same blood draw for the ratio to mean anything.
What the ratio is actually about
Aromatase converts testosterone into estradiol, so the T/E2 ratio is sometimes used as a rough proxy for aromatization activity — higher body fat, for instance, carries more aromatase. Research has linked the ratio (especially with free testosterone) to outcomes like libido in cohort studies, but it is not a validated clinical marker with official cutoffs.
Evidence level & limitations
The 10–30 window is a men's-health community convention — no endocrine society defines a target T/E2 ratio, and individual labs report their own reference ranges for each hormone separately. Cycle timing (for women), age, body composition, medications (including aromatase inhibitors and TRT), and assay method all move the inputs. Men's-health communities commonly cite 10–30 (ng/dL per pg/mL) as a typical balance for adult men. Be aware that no endocrinology guideline defines a target ratio — this is a community convention, and both too-high and too-low estradiol relative to testosterone are discussed as patterns worth a clinician's input, not self-treatment targets.
Does the T/E2 ratio apply to women?
Not with the same numbers. Women have far lower testosterone and cyclical estradiol, so the 10–30 window doesn't translate — and cycle phase changes estradiol several-fold. Is higher always better for the ratio?
No — and this is the most common misreading. Estradiol is essential for men too: bone density, libido, joint comfort, and cardiovascular health all depend on it. Chasing a very high ratio (i.e., very low estradiol) creates its own problems. The goal is balance, not maximization — and any adjustment belongs with a clinician.
My ratio is outside the window — what should I actually look at?
Start with the two values individually against your lab's own reference ranges, plus free testosterone and SHBG if you have them — the ratio is a secondary view, not a standalone answer. Symptoms and clinical history matter more than any single computed number; take the full picture to a qualified practitioner.
Can I use this with saliva or urine (DUTCH) results?
The unit math works, but the 10–30 convention comes from serum-based community data — saliva and urine metabolite levels are numerically very different, so the window doesn't transfer. If you're working from a DUTCH or saliva panel, interpret each value within that panel's own reference ranges instead.
