VLDL (very-low-density lipoprotein) is rarely measured directly — labs estimate it from your triglycerides. The same standard lipid panel also lets you derive LDL with the Friedewald equation, which is exactly how most lab reports produce their 'calculated LDL' line. How it works

The formulas

VLDL-C ≈ triglycerides ÷ 5 (mg/dL), the Friedewald estimate of the cholesterol carried in VLDL particles. LDL-C = total cholesterol − HDL − (triglycerides ÷ 5), the Friedewald equation published in 1972 and still the basis of the 'calculated LDL' on most lab reports. In mmol/L the VLDL estimate is triglycerides ÷ 2.2.

When the Friedewald estimate breaks down

The TG÷5 assumption fails when triglycerides are 400 mg/dL or higher, in non-fasting samples, and in rare type III dysbetalipoproteinemia. It also tends to underestimate LDL when LDL is very low and triglycerides are elevated — newer equations (Martin/Hopkins, Sampson) or a direct LDL measurement handle those cases better.

What VLDL reflects

VLDL particles are the liver's triglyceride delivery trucks. High estimated VLDL usually means high triglycerides and a heavier load of remnant particles — a cardiometabolic signal that pairs naturally with fasting insulin and non-HDL cholesterol. VLDL reference bands are laboratory conventions, not official guideline cut-offs — no major guideline issues VLDL-C thresholds. The LDL bands follow the ATP III categories. Both estimates are computed from routine labs and are screening signals, not diagnoses.

Frequently asked questions

How is VLDL cholesterol calculated?

By dividing your fasting triglycerides by 5 (in mg/dL) — the Friedewald estimate most labs use. Example: triglycerides of 150 mg/dL give an estimated VLDL-C of 30 mg/dL. It is an estimate, not a direct measurement.

What is a normal VLDL level?

Most lab references put normal estimated VLDL-C at roughly 2–30 mg/dL. These are laboratory conventions — no major guideline issues official VLDL cut-offs, and the number only makes sense alongside your triglycerides and non-HDL cholesterol.

My report says 'VLDL calculated high' — what does that mean?

Almost always that your triglycerides are elevated, since VLDL is estimated as triglycerides ÷ 5. It is a screening signal pointing at triglyceride-rich particles — worth a fuller look (fasting insulin, non-HDL-C, ApoB) with your practitioner, not a diagnosis on its own.

When is the Friedewald formula not valid?

When triglycerides are 400 mg/dL or more, when the sample isn't fasting, and in rare type III dysbetalipoproteinemia. It also underestimates LDL when LDL is very low with high triglycerides — in those cases a direct LDL measurement or a newer equation (Martin/Hopkins, Sampson) is more accurate.

What's the difference between calculated LDL and direct LDL?

Calculated LDL is derived from total cholesterol, HDL, and triglycerides via the Friedewald equation — it's the default on most reports because it's free. Direct LDL is a separate lab assay that measures LDL cholesterol itself, useful when the calculation is unreliable (very high triglycerides, non-fasting samples).

How is this different from cholesterol ratios?

VLDL and LDL are absolute cholesterol values estimated from your panel; ratios (TC/HDL, LDL/HDL on our Cholesterol Ratio Calculator, or TG/HDL on its own page) express the balance between two values. They complement each other — ratios summarize proportion, VLDL/LDL give absolute load.
