How it works
The formula
WHtR = waist circumference ÷ height. Because it is a ratio, the units cancel out — inches with inches, centimeters with centimeters, the result is the same. Measure your waist at the level of the navel, standing relaxed, at the end of a normal exhale.
Why not just use BMI?
BMI cannot distinguish fat from muscle, and it says nothing about where fat is stored. Fat concentrated around the abdomen is metabolically different — more strongly linked to insulin resistance and cardiovascular risk in large cohort studies. Several analyses have found WHtR outperforms both BMI and waist circumference alone as a screening marker for cardiometabolic risk.
Limitations
WHtR is a population-level screening heuristic, not an individual assessment. It is less informative during pregnancy, in very muscular builds, and at the extremes of height. Cut-offs like 0.5 are rules of thumb, not diagnostic thresholds — trends and context matter more than any single reading.

What is a healthy waist-to-height ratio?
For most adults, a ratio between 0.4 and 0.5 is generally considered healthy. Above 0.5, research links central adiposity to higher cardiometabolic risk — hence the widely cited advice to keep your waist to less than half your height.
Is 0.5 a universal cut-off?
It is a well-supported rule of thumb across many populations, but not a precise boundary. Risk rises gradually rather than switching on at 0.5, and optimal values may vary somewhat by age, sex, and ethnicity.
How do I measure my waist correctly?
Stand relaxed, find the level of your navel, and wrap a tape measure snugly — not tight — around your bare waist at the end of a normal exhale. Use the same unit (inches or centimeters) for both waist and height.
Can I use centimeters instead of inches?
Yes. The ratio is unit-agnostic — 81 cm waist and 170 cm height gives exactly the same result as 32 in and 67 in. Just never mix units in the same calculation.
Is a high waist-to-height ratio a diagnosis?
No. WHtR is a simple screening marker. A high value is a prompt to look at the fuller picture — blood pressure, lipids, fasting glucose — with a qualified practitioner, not a diagnosis of any condition.
